Terms & Conditions
1. The customers have to present the certificate of identity (such as HKID card, passport) and the order confirmation e-mail for identification.
2. Reservation is recommended to make at least 3-4 days in advance after purchasing the checkup package.
3. Staff of clinic would provide the receipt if necessary. The name printed / written on the receipt has to be the same as the patient who do the service.
4. Amendment or cancellation is not allowed once the payment is confirmed. Medical certificate or assessment by our doctor must be presented as support document if any change of the checkup package or cancellation is needed. If refund is necessary, minimum HK$100 or 3.5% of the amount of the checkup package would be charged as administration fee. If we cannot provide the service, total amount of the order would be refunded and no administration fee would be charged.
5. The customers have to contact us for making appointment after successful payment & receiving the order confirmation e-mail.

6. Please contact the clinic for any change of the appointment.
7. The order would be valid for six months from the date of purchase, the customers have to make the appointment and complete the service before the deadline. The order would be forfeited when it is expired.
8. Neither amendment nor cancellation is allowed once the payment has been confirmed. Also, it is not transferable.
9. Our service would be affected when the typhoon signal no. 8 or above & Black Rainstorm Warning is issued, please check the latest announcement on our website, Facebook or Instagram Fan Page.
10. Preparations for different health services are varied, please contact us for details.
11. WhatsApp notification would be sent when the report is ready.
12. The customers have to pick up the health check report within 1 month after the date of checkup. We reserve the right to erase the checkup report without any prior notice if the customers cannot pick up the report before the deadline.
13. Presentation of the authorization letter & copy of the patient’s HKID card is needed if the health check report would be picked up by the third party.
14. All tests are not for the purpose of medical diagnosis or treatment. Please enquire your doctor for any questions.
15. Swindon Medical reserves all rights to change the terms and conditions and to invalidate the offer without prior notification.
16. These Terms of Service and any separate agreements whereby we provide you shall be governed by and interpreted in accordance with the laws of Hong Kong, China.
